RESUMO

Respiratory failure may affect up to 1 in 500 pregnancies, due to pregnancy-specific conditions, conditions aggravated by the pregnant state, or other causes. Management during pregnancy is influenced by altered maternal physiology, and the presence of a fetus influencing imaging, and drug therapy choices. Few studies have addressed the approach to invasive mechanical ventilatory management in pregnancy. Hypoxemia is likely harmful to the fetus, but precise targets are unknown. Hypocapnia reduces uteroplacental circulation, and some degree of hypercapnia may be tolerated in pregnancy. Delivery of the fetus may be considered to improve maternal respiratory status but improvement does not always occur.

Objetivo: Evaluar la evolución de la musculatura perineal en mujeres primigestas afectas de obesidad (índice de masa corporal [IMC]≥30) y comparar dicha evolución con mujeres primigestas con un IMC en rango de normopeso. Método: Se realizó un estudio cohorte longitudinal prospectivo donde se estudió a las gestantes en 3 momentos: en semana 12 de gestación, la semana 34 de gestación y a las 12 semanas tras el parto. En estas consultas se realizó medición mediante perinometría para determinar el tono basal (TB), la fuerza máxima contráctil (FMax); y la fuerza ejercida (FE). Además, se utilizó el test de Oxford modificado (MOS). Para el estudio estadístico se utilizaron modelos estadísticos generales mixtos. Resultados: Se reclutaron al inicio 50 gestantes nulíparas (25 con IMC≥30 y 25 con IMC<25), concluyendo el estudio completo 39. Al inicio del embarazo el tono perineal basal fue de 4,62±0,24Newton (Nw) y descendió a 4,18±0,26Nw tras el parto. La capacidad contráctil fue de 5,56±0,79Nw en la semana 12 y aumento a 6,34±1,24Nw tras el parto. Al comparar la FCMax en la semana 12 en gestantes obesas vs. Normopeso se observan valores de 5,51±87Nw vs. 5,61±0,71Nw (p=0,941). Se observaron valores posparto 6,72±1,17Nw vs. 5,95±1,21Nw (p=0,024), superiores en la población con obesidad. Conclusiones: Existe una disminución significativa del tono basal que es contrarrestada con un aumento de la fuerza contráctil objetivable a las 12 semanas posparto. El organismo dispone de mecanismos compensadores que permiten la recuperación a partir de los 3 meses posparto existiendo una mayor fuerza muscular dentro de las pacientes con obesidad.(AU)


Objective: Evaluating the changes of the perineal muscles in primigravid women with obesity (BMI≥30) and to compare the progress with primigravid women in normal BMI range. Methods: A prospective longitudinal cohort study was carried out. The pregnant women were studied at three moments: at 12 weeks’ gestation, at 34 weeks’ gestation and at 12 weeks after delivery. During the ckeck-up, perinometric measurements were taken to determine basal tone (BT), maximum contractile force (FMax) and applied forced (AF). In addition, the Oxford test (MOS) was used. General statistical mixed models were used for the statistical study. Results: Fifty nulliparous pregnant women (25 with BMI≥30 and 25 with BMI<25) were recruited at the beginning of the study and 39 completed the entire study. Basal tone (BT) was 4.62±0.24 Nw at the beginning of pregnancy and decreased to 4.18±0.26 Nw after delivery. Contractile capacity was 5.56±0.79 Nw at week 12 and increased to 6.34±1.24 Nw after delivery. When comparing the FCMax at week 12 in obese vs. normal weight pregnant women, values of 5.51±87 Nw vs. 5.61±0.71 Nw were observed (P=.941). Postpartum values were 6.72±1.17 Nw vs. 5.95±1.21 Nw (P=.024), higher in the obese population. Conclusions: There is an increase in contractile strength (Fmax) at 12 weeks postpartum in order to counteract the significant decrease in basal tone (BT). The body has compensatory mechanisms that allow recovery after 3 months postpartum, with greater muscle strength in obese patients.(AU)

AIM: To determine the association of successful and unsuccessful operative vaginal delivery attempts with risk of severe neonatal birth injury. METHODS: We conducted a population-based observational study of 1 080 503 births between 2006 and 2019 in Quebec, Canada. The main exposure was operative vaginal delivery with forceps or vacuum, elective or emergency cesarean with or without an operative vaginal attempt, and spontaneous delivery. The outcome was severe birth injury, including intracranial hemorrhage, brain and spinal damage, Erb's paralysis and other brachial plexus injuries, epicranial subaponeurotic hemorrhage, skull and long bone fractures, and liver, spleen, and other neonatal body injuries. We determined the association of delivery mode with risk of severe birth injury using adjusted risk ratios (RR) and 95% confidence intervals (CI). RESULTS: A total of 8194 infants (0.8%) had severe birth injuries. Compared with spontaneous delivery, vacuum (RR 2.98, 95% CI 2.80-3.16) and forceps (RR 3.35, 95% CI 3.07-3.66) were both associated with risk of severe injury. Forceps was associated with intracranial hemorrhage (RR 16.4, 95% CI 10.1-26.6) and brain and spinal damage (RR 13.5, 95% CI 5.72-32.0), while vacuum was associated with epicranial subaponeurotic hemorrhage (RR 27.5, 95% CI 20.8-36.4) and skull fractures (RR 2.04, 95% CI 1.86-2.25). Emergency cesarean after an unsuccessful operative attempt was associated with intracranial and epicranial subaponeurotic hemorrhage, but elective and other emergency cesareans were not associated with severe injury. CONCLUSIONS: Operative vaginal delivery and unsuccessful operative attempts that result in an emergency cesarean are associated with elevated risks of severe birth injury.

INTRODUCTION: Health facility preparedness is essential for delivering quality maternal and newborn care, minimizing morbidity and mortality by addressing delays in seeking skilled care, reaching appropriate facilities, and receiving emergency care. A rapid assessment of 23 government health facilities in Kilifi and Kisii counties identified poor maternal and newborn indicators in 16 facilities. The Access to Quality Care through Extending and Strengthening Health Systems (AQCESS) project supported these facilities with training, equipment, and referral linkages. This study focuses on facility preparedness of the 16 facilities to deliver maternal and newborn health services, specifically delays two and three at the end of the project implementation. METHODS: A descriptive cross-sectional study was carried-out on behalf of AQCESS project team by respective county ministry of health in-charge of reproductive maternal newborn and child health programs and trained nurses and medical doctors from Aga Khan health services in December 2019. The study evaluated the accessibility and reliability of drugs, commodities, equipment, personnel, basic necessities (such as water and electricity), and guidelines using validated World Health Organization service availability and readiness assessment tool. The findings of the assessment are presented through frequency and percentage analysis, along with a comparative analysis between the two counties. RESULTS: All the 16 facilities assessed offered routine antenatal care (ANC) and normal delivery, but only two provided comprehensive emergency obstetric and newborn care (CEmONC). Most essential medicines, commodities, and required equipment were available. BEmONC and CEmONC guidelines were present in Kilifi, not in Kisii. One staff member was available 24/7 for cesarean section (CS) in each county, with one anesthetist in Kilifi. Electricity was accessible in all facilities, but only half had secondary power supply. Facilities offering CS had backup generators. CONCLUSION: The Facilities assessed had necessary drugs, commodities, equipment, and requirements, but staffing and guidelines were limited. Kilifi outperformed Kisii in most indicators. Additional support is needed for infrastructure and human resources to deliver quality maternal and newborn health services. Continuous monitoring will facilitate resource allocation based on facility needs.

OBJECTIVE: The series of interventions that comprise labor induction shape patient experiences; however, patient perceptions are not always considered when structuring the process of care. Through qualitative interviews, we elucidated women's expectations and experiences regarding labor induction. METHODS: Labor induction patients were recruited from a United States tertiary care hospital's postpartum mother-baby unit and invited to participate in semi-structured qualitative interviews. Interview questions included expectations and experiences of the labor induction process, side effects and health outcomes of concern, reflections on personal tolerance of different interventions, and thoughts about an ideal process. RESULTS: Between April and September 2018, 36 women were interviewed. The labor induction process involved a wide range of experiences; when asked to characterize labor induction in one word, responses ranged from horrible, frustrating, and terrifying to simple, fast, and smooth. Inductions were often described as longer than what was expected. The most polarizing induction method was the Foley balloon catheter. Women's concerns regarding side effects largely centered on the health of their baby, and an ideal induction involved fewer interventions. CONCLUSIONS: Experiences with labor induction vary greatly and are related to expectations. The way interventions are introduced influences women's perceptions of control and their ultimate level of contentment with the birthing process. Attention to experiences and preferences has the potential to improve quality of care through communication, shared decision-making, and education.

PROBLEM: A lack of consistent professional guidance on when to intervene during emergence (buttocks and anus visible at the introitus to birth of the head) in vaginal breech birth (VBB). BACKGROUND: Hypoxia and asphyxia are common complications of VBB, especially due to umbilical cord compression around the time of emergence. AIM: To gain insight into VBB time management trends, the evidence behind these practices and how they may have influenced outcomes. METHODS: Literature review of obstetric textbooks published between 1960 and 2000 contained in the Wellcome Collection and Royal College of Obstetricians and Gynaecologists Library in London. FINDINGS: 90 textbooks were reviewed. Recommendations for 'safe' intervals between birth of the umbilicus and the head ranged from 5 to 20 min. Many sources focused only on the time required to deliver the head, with 'up to 10 min' being the most common interval described. The review found no mention of cord compression causing concern earlier in breech births than once the umbilicus itself is delivered, nor any evidence to support the recommendations. DISCUSSION: These findings demonstrate a long-term pattern across the second half of the 20th century, whereby birth attendants were urged not to rush emergence nor delay intervention but were given little clear guidance on optimal timings. CONCLUSION: Clear, evidence-based guidance should be provided in breech training materials to avoid unnecessary hypoxic injuries, and this guidance should be rigorously evaluated.

Background: The rates of repeat cesarean section (CS) among women with previous CS are increasing worldwide. The predictors of a repeat CS can vary across different populations. Objective: To determine the predictors of repeat CS among women from Malaysia with one previous lower segment CS (LSCS) who underwent trial of labor (TOLAC). Materials and Methods: This retrospective cohort study included women with one previous LSCS who followed up and delivered their current pregnancy at Hospital Universiti Sains Malaysia (USM), Kelantan, Malaysia, between January 01, 2016, and December 31, 2017. Women with singleton pregnancies were included while those who had a history of classical CS, current pregnancy with preterm birth, non-cephalic pregnancy, lethal fetal anomalies, uterine rupture, and severe preeclampsia or planned for elective CS were excluded. Logistic regressions were performed. Results: The study included 388 women who underwent TOLAC and successfully gave childbirth through vaginal birth after cesarean (VBAC) (n = 194) or emergency LSCS (n = 194). Factors significantly associated with repeat CS were no history of vaginal delivery (adjusted odds ratio (aOR): 2.71; 95% confidence interval (CI): 1.60, 4.60; P < 0.001), estimated fetal weight ≥3500 grams (aOR: 4.78; 95% CI: 2.45-9.34; P < 0.001), and presence of meconium-stained liquor (aOR: 2.40; 95% CI: 1.33-4.35; P = 0.004). Conclusion: The above-mentioned predictors of a repeat CS among women from Malaysia with one previous LSCS who underwent TOLAC can be useful for clinicians in making an informed decision.

Objetivo: Avaliar a taxa de cesáreas e suas principais indicações com base na classificação de Robson na Maternidade Municipal de São Vicente em 2020, um hospital público de risco habitual. Métodos: Trata-se de um estudo transversal observacional. Foram efetuadas revisão, correção e análise retrospectiva e documental da classificação de Robson na Maternidade Municipal de São Vicente. Foram analisados partos de janeiro a dezembro de 2020, dos quais foram coletadas e ordenadas as informações mais relevantes para a pesquisa. Resultados: Uma amostra de 1.627 partos foi encontrada. A taxa geral de cesáreas encontrada foi de 46,3%. A contribuição relativa dos grupos 1, 2 e 5 para a taxa de cesáreas foi de 16,8%, 13,3% e 46,8%, respectivamente, enquanto a contribuição relativa das indicações de cesáreas foi de 25,5% para parto cesáreo anterior e de 21,5% para sofrimento fetal agudo. Conclusão: Foi evidenciada alta taxa de cesáreas, e as principais indicações foram cesárea prévia e sofrimento fetal agudo. Os grupos 1, 2 e 5 da classificação de Robson foram os que mais contribuíram para essa taxa.


Objective: To evaluate the cesarean section rate and the cesarean indication rate based on Robson Classification during 2020 in Sã o Vicente's Municipal Maternity, a habitual-risk public hospital. Methods: This is a cross-sectional observational study. We have reviewed, corrected, analyzed retrospectively and documented Robson Classification in Sã o Vicente's Municipal Maternity. Births from January to December 2020 were analyzed, from which the main data for the research was collected and organized. Results: A sample of 1,627 births was found. The overall rate of cesarean section was 46.3%. The relative contribution of groups 1, 2 and 5 to the cesarean rate was 16.8%, 13.3% and 46.8%, respectively. While the cesarean indication relative contribution was 25.5% for previous cesarean and 21.5% for fetal distress. Conclusion: We found a high cesarean rate and the main indications were previous cesarean and fetal distress. Robson classification groups 1, 2 and 5 contributed the most to this rate.

INTRODUCTION: We identified risk factors and outcomes associated with SARS-CoV-2 infection in pregnancy in a universally tested population according to disease severity and validated information on SARS-CoV-2 during pregnancy in national health registers in Denmark. MATERIAL AND METHODS: Cohort study using data from national registers and medical records including all pregnancies between March 1, 2020 and February 28, 2021. We compared women with a validated positive SARS-CoV-2 test during pregnancy with non-infected pregnant women. Risk factors and pregnancy outcomes were assessed by Poisson and Cox regression models and stratified according to disease severity defined by hospital admission status and admission reason (COVID-19 symptoms or other). Using medical record data on actual period of pregnancy, we calculated predictive values of the SARS-CoV-2 diagnosis in pregnancy in the registers. RESULTS: SARS-CoV-2 infection was detected in 1819 (1.6%) of 111 185 pregnancies. Asthma was associated with infection (relative risk [RR] 1.63, 95% confidence interval [CI] 1.28-2.07). Risk factors for severe COVID-19 disease requiring hospital admission were high body mass index (median ratio 1.06, 95% CI 1.04-1.09), asthma (RR 7.47, 95% CI 3.51-15.90) and gestational age at the time of infection (gestational age 28-36 vs < 22: RR 3.53, 95% CI 1.75-7.10). SARS-CoV-2-infected women more frequently had hypertensive disorders in pregnancy (adjusted hazard ratio [aHR] 1.31, 95% CI 1.04-1.64), early pregnancy loss (aHR 1.37, 95% CI 1.00-1.88), preterm delivery before gestational age 28 (aHR 2.31, 95% CI 1.01-5.26), iatrogenically preterm delivery before gestational age 37 (aHR 1.49, 95% CI 1.01-2.19) and small-for-gestational age children (aHR 1.28, 95% CI 1.05-1.54). The associations were stronger among women admitted to hospital for any reason. The validity of the SARS-CoV-2 diagnosis in relation to pregnancy in the registers compared with medical records showed a negative predictive value of 99.9 (95% CI 99.9-100.0) and a positive predictive value of 82.1 (95% CI 80.4-83.7). CONCLUSIONS: Women infected with SARS-CoV-2 during pregnancy were at increased risk of hypertensive disorders in pregnancy, early pregnancy loss, preterm delivery and having children small for gestational age. The validity of Danish national registers was acceptable for identification of SARS-CoV-2 infection during pregnancy.

Introduction: There has been a global decrease in operative vaginal deliveries, with a marked shift towards the vacuum extractor. However, little is known about the trends in operative vaginal delivery in Singapore. Methods: A retrospective study was conducted on all operative vaginal deliveries performed from 2012 to 2017 at Singapore General Hospital (SGH). Maternal outcomes in terms of postpartum haemorrhage and obstetric anal sphincter injuries were compared between forceps- and vacuum-assisted deliveries. Neonatal outcomes in terms of neonatal intensive care unit (NICU) admission and clinically significant neonatal events were compared. The instrument preference of obstetricians was analysed. Results: A total of 906 consecutive operative vaginal deliveries were included in the study, comprising 461 forceps- and 445 vacuum-assisted deliveries. The rate of operative vaginal delivery was maintained at approximately 10% from 2012 to 2017. Neonatal cephalohematomas were more common after vacuum-assisted deliveries. Other maternal and neonatal outcomes did not differ significantly between the two groups. Clinically significant neonatal events were mostly due to shoulder dystocia, whereas all cases of NICU admissions were not directly related to the mode of delivery. Obstetricians' choice of instrument appeared to reflect personal preference and was not affected by the year of graduation. Conclusion: The rates of neonatal and maternal morbidity were low at SGH. Overall instrument use of forceps and vacuum was balanced, and proficiency in both was demonstrated by all operators. Operative vaginal delivery remains an essential skill in facilitating safe vaginal delivery, which should be maintained to keep Caesarean section rates in check.

RESUMO Objetivo: Verificar a prevalência e identificar os fatores associados à ausência do acompanhante de parto em mulheres no sul do Brasil. Métodos: Trata-se de um estudo transversal, realizado com 466 parturientes, pertencentes a uma coorte de mulheres da zona urbana da cidade de Pelotas, RS. Aos 18 meses pós-parto, foi aplicado um questionário estruturado com dados sociodemográficos, gestacionais e questões relacionadas ao parto. Foi realizada regressão logística para ajustes de possíveis fatores de confusão. Resultados: A prevalência da ausência de acompanhante de parto entre as mulheres foi de 22,3%. As parturientes com até 8 anos de estudo (RP=2,0 [IC95% 1,1-3,8]), que não viviam com um companheiro (RP=2,3 [IC95% 1,2-4,3]), que realizaram o pré-natal no setor público (RP=1,9 [IC95% 1,0-3,7]) e que tiveram um parto via cesárea (RP=6,0 [IC95% 2,9-12,4]) apresentaram maior probabilidade de ausência de acompanhante de parto. Conclusão: Os resultados apontam evidências relevantes para o seguimento da verificação da presença do acompanhante de parto no sul do Brasil, indicando a necessidade de melhor aproveitamento e adesão desta prática. Além disso, a lei que aprova a presença do acompanhante de parto no Brasil parece não estar sendo colocada em prática de modo integral, desrespeitando um direito das parturientes e impactando nos benefícios para a saúde materno-infantil.


ABSTRACT Objective: To verify the prevalence and identify the factors associated with the absence of birth companions among women in Southern Brazil. Methods: This is a cross-sectional study carried out with 466 parturient women in a cohort of women from the urban area of the city of Pelotas, RS. At 18 months postpartum, a structured questionnaire was applied with sociodemographic, gestational data and questions related to childbirth. Logistic regression was performed to adjust for possible confounding factors. Results: The prevalence of the absence of a birth companion among women was 22.3%. Parturient women with up to 8 schooling years (PR=2.0 [95%CI 1.1-3.8]), who did not live with a partner (PR=2.3 [95%CI 1.2-4.3]), who performed their prenatal care in the public sector (PR=1.9 [95%CI 1.0-3.7]) and who had a cesarean delivery (PR=6.0 [95%CI 2.9-12.4]) were more likely to not have had a birth companion. Conclusion: The results shows relevant evidence for the verification of the presence of a companion in Southern Brazil, indicating the need for better use and adherence to this practice. In addition, the law that approves the presence of the birth companion in Brazil does not seem to be being fully implemented, disrespecting a right of parturient women and impacting the benefits for for maternal and child health.

SUMMARY OBJECTIVE: The aim of this study was to examine the impact of symptom-based screening on the prevalence and outcomes of neonatal coronavirus disease 2019 in pregnant women admitted for delivery. METHODS: A retrospective observational study was conducted from June to August 2020 at Gonzaga Mota of Messejana Hospital, Fortaleza, CE, Brazil. All pregnant women were screened for coronavirus disease 2019 based on symptoms. Reverse transcription-polymerase chain reaction or immunology assays for severe acute respiratory syndrome coronavirus 2 were performed when a patient reported a symptom. All newborns of symptomatic patients were submitted for Reverse transcription-polymerase chain reaction. Newborns were divided into groups according to the Reverse transcription-polymerase chain reaction results to identify the relationship between maternal symptoms and neonatal coronavirus disease 2019. RESULTS: A total of 55 (55/1,026, 5.4%) and 50 (50/1,026, 4.8%) pregnant women reported symptoms and had a positive confirmatory test, respectively. The most common symptom of coronavirus disease 2019 among the pregnant women with positive confirmatory test was cough (n=23, 46%). Seven newborns (7/50, 14%) of symptomatic mothers had positive Reverse transcription-polymerase chain reaction. Upon birth, no newborn had serious complications. CONCLUSION: Universal screening of pregnant women admitted for delivery can reduce the perinatal transmission of coronavirus disease 2019. Symptom-based screening can be an alternative for regions with a low prevalence of the disease where a better allocation of financial resources is necessary.

BENEFITS, HARMS, AND COSTS: Appropriate recognition and treatment of postpartum hemorrhage can prevent serious morbidity while reducing costs to the health care system by minimizing more costly interventions and length of hospital stays. EVIDENCE: Medical literature, PubMed, ClinicalTrials.gov, the Cochrane Database, and grey literature were searched for articles, published between 2012 and 2021, on postpartum hemorrhage, uterotonics, obstetrical hemorrhage, and massive hemorrhage protocols. VALIDATION METHODS: The authors rated the quality of evidence and strength of recommendations using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. See online Appendix A (Tables A1 for definitions and A2 for interpretations of strong and conditional [weak] recommendations). INTENDED AUDIENCE: All members of the health care team who care for labouring or postpartum women, including, but not restricted to, nurses, midwives, family physicians, obstetricians, and anesthesiologists.

Objective: Determining the appropriate approach for delivery after previous cesarean is a very controversial issue. Our objective was to establish whether pregnant women with a previous cesarean have an increased maternal and fetal morbidity and mortality after attempting vaginal delivery as well as to determine which factors may influence the achievement of a vaginal birth after cesarean. Materials and methods: A retrospective observational cohort study including 390 patients (196 cesarean group and 194 nulliparous group) was carried out. We compared neonatal and maternal outcomes between groups. Afterward, a multivariate logistic regression was applied for our second objective. Results: There were higher rates of uterine rupture (2% vs. 0%, p: 0.045) and puerperal hemorrhage (9.7% vs. 3.1%, p: 0.008) in the cesarean group and lower vaginal delivery rate (58.2% vs. 77.8%, p < 0.0005). We found that the induced onset of labor (OR = 2.9) and new born weight (OR = 1.0001) were associated with an increased risk of cesarean section. Conclusions: Our findings stress the need for further investigations in this field, which might provide a basis for a better management of patients with a previous cesarean.


Objetivo: Determinar el abordaje adecuado del tipo de parto tras una cesárea previa es un tema muy controvertido. Nuestro objetivo fue establecer si las gestantes con cesárea previa presentan mayor morbimortalidad materna y fetal tras intentar parto vaginal, así como determinar qué factores pueden influir en conseguir un parto vaginal posterior a la cesárea. Material y métodos: Estudio observacional de cohortes retrospectivo incluyendo 390 pacientes (196 con cesárea previa, 194 nulíparas). Comparamos los datos sobre los resultados neonatales y maternos. Posteriormente se aplicó un modelo de regresión logística multivariante. Resultados: Hubo mayores tasas de ruptura uterina (2% vs. 0%; p = 0.045) y hemorragia puerperal (9.7% vs. 3.1%, p: 0.008) en el grupo de cesárea anterior, así como una tasa de parto vaginal mas baja (58.2% vs. 77.8%, p < 0.0005). La inducción del parto (OR = 2,9) y el peso del recién nacido (OR = 1.0001) se asociaron a un mayor riesgo de cesárea. Conclusión: La probabilidad de parto vaginal en estas pacientes disminuye cuanto mayor sea el peso del recién nacido y con partos inducidos.

OBJECTIVE: the management of the second stage of labor and its optimal duration are controversial, particularly for nulliparous women. Our aim is to analyze the impact of a prolonged second stage on perinatal morbidity in our institution. MATERIAL AND METHODS: this is a retrospective study conducted in the University Hospital of Liège (Belgium) from January to July 2019. The 160 patients included were divided into two groups according to the duration of the second stage and compared in terms of mode of delivery, maternal and neonatal morbidity. RESULTS: group 1 (85 %) had a second stage of labor inferior to 3 hours and group 2 (15 %) ? 3 hours. The cesarean deliveries for non-engagement of the fetus were significantly higher in group 2. No significant difference was observed in terms of maternal morbidity. The median Apgar score at 5 minutes was higher in group 1. The proportion of babies transferred to neonatal intensive care and to the neonatology department was higher in group 2. CONCLUSION: we mainly noted an increase in neonatal morbidity when the second stage exceeded 3 hours, which is consistent with literature where an increase in maternal morbidity is also described. Prolonging the second stage therefore seems inappropriate to us in the current state of knowledge.


OBJECTIFS: la gestion du second stade de travail et sa durée optimale sont controversées, particulièrement pour les patientes nullipares. Notre objectif est d'étudier l'impact d'un second stade prolongé sur la morbidité périnatale dans notre institution. Matériel et méthodes : il s'agit d'une étude rétrospective menée au CHU de Liège de janvier à juillet 2019. Les 160 patientes incluses ont été réparties en deux groupes en fonction de la durée du second stade et comparées en termes de mode d'accouchement, morbidité maternelle et néonatale. Résultats : le groupe 1 (85 %) présentait un second stade inf�rieur a 3 heures et le groupe 2 (15 %) ? 3 heures. Les césariennes pour non-engagement étaient significativement plus élevées dans le groupe 2. Aucune différence significative n'a été observée en termes de morbidité maternelle. Le score d'Apgar médian à 5 minutes était plus élevé dans le groupe 1. La proportion de bébés transférés au NIC («Neonatal Intensive Care¼) et en néonatologie était plus élevée dans le groupe 2. CONCLUSION: nous avons principalement noté une majoration de la morbidité néonatale lorsque le second stade dépassait 3 heures, ce qui rejoint la littérature où l'on décrit, également, une majoration de la morbidité maternelle. Prolonger le second stade nous semble, dès lors, inapproprié dans l'état actuel des connaissances.

Introducción: La miastenia gravis (MG) se caracteriza por debilidad del músculo esquelético, que es la presentación inicial más común, debilidad ocular con ptosis asimétrica y diplopía binocular. Alrededor del 19-50% de las gestantes con MG experimentará un empeoramiento de la enfermedad. El objetivo de este artículo fue revisar la información actual respecto a la interrelación entre la MG y el embarazo, así como su abordaje. Desarrollo: Se realizó una búsqueda bibliográfica en bases de datos como PubMed, ScienceDirect, SciELO, Google Scholar y medRxiv. Se incluyeron artículos originales, de revisión, series e informes de casos comprendidos entre 2013 y 2022. Conclusiones: La MG no afectaría significativamente al embarazo; sin embargo, el embarazo puede exacerbar la enfermedad, principalmente durante el primer trimestre o después del parto. El abordaje de la gestante con MG debe ser multidisciplinario e implica el ajuste del tratamiento farmacológico y un seguimiento constante.(AU)


Introduction: Myasthenia gravis is characterized by skeletal muscle weakness, the most common initial presentation includes ocular weakness with asymmetric ptosis and binocular diplopia. Around 19-50% of pregnant women with myasthenia gravis will experience a worsening of the disease. The objective of this article was to review the current information regarding the interrelation between MG and pregnancy; as well as its approach. Development: Bibliographic search in databases such as PubMed, ScienceDirect, SciELO, Google Scholar and medRxiv. Original articles, reviews, series and case reports between 2013 and 2022 are included. Conclusions: Myasthenia gravis would not significantly effects on pregnancy; however, pregnancy can exacerbate the disease, especially during the first trimester or after delivery. The approach to pregnant women with myasthenia gravis must be multidisciplinary and involves the adjustment of pharmacological treatment and constant monitoring.(AU)

INTRODUCTION: Obstetric anal sphincter injuries (OASIS) are severe complications to vaginal births with potentially serious long-term consequences and large impact on quality of life. The aim was to determine risk and protective factors for OASIS. MATERIAL AND METHODS: We performed a retrospective register-based observational study. A cohort of 988 988 singleton term deliveries 2005-2016 in Sweden were included. Data from the Swedish Medical Birth Registry and Statistics Sweden were extracted to identify cases of OASIS and maternal and fetal characteristics. Modified Poisson Regression analyses were performed to assess risk factors. RESULTS: The rate of OASIS was 3.5% (n = 34 583). Primiparity (adjusted risk ratio [aRR] 3.13, 95% CI 3.05-3.21), vacuum extraction (aRR 2.79, 95% CI 2.73-2.86), forceps (aRR 4.27, 95% CI 3.86-4.72), and high birthweight (aRR 2.61, 95% CI 2.50-2.72) were associated with a significantly increased risk of OASIS. Increasing maternal age and decreasing maternal height also increased the risk of OASIS. Obesity increased the risk of OASIS (aRR 1.04, 95% CI 1.04-1.08), if fetal birthweight was not adjusted for. Smoking (aRR 0.74, 95% CI 0.70-0.79) and low maternal education (aRR 0.87, 95% CI 0.83-0.92) were associated with a decreased frequency of reported OASIS. Previous cesarean section increased the risk of OASIS (aRR 1.41, 95% CI 1.36-1.47). CONCLUSIONS: Primiparity, instrumental delivery, and high birthweight significantly increased the risk of OASIS. Obesity, low height, increasing age, and previous cesarean section also increased the risk whereas smoking and low maternal educational level were associated with a lower OASIS rate.

Introducción: Potenciar la participación de las mujeres durante el embarazo y el parto se alinea con el llamado de la Organización Mundial de la Salud y se vincula con efectos en la satisfacción usuaria, resultados clínicos de salud y una mejor gestión de los prestadores de salud. Objetivo : Descubrir las necesidades de participación en la toma de decisiones de las mujeres durante el proceso del embarazo y parto. Método : Análisis secundario de un estudio cualitativo descriptivo con mujeres hospitalizadas del servicio de puerperio de dos hospitales en Santiago de Chile. El análisis de los datos se realizó utilizando el método propuesto por la Grounded Theory. Resultados : Participaron doce mujeres en dos grupos focales. Del análisis relacional se desprende que la participación en el proceso reproductivo es interferida por dos grupos de factores vinculados a significados culturales y a la vulneración de los derechos de las mujeres. Conclusiones. La participación de las mujeres en las decisiones clínicas durante el proceso de embarazo y parto es aún escasa y el poder sigue manteniéndose en los profesionales de la salud, perpetuándose prácticas de violencia institucional. Para avanzar en una práctica obstétrica centrada en las mujeres, es importante reconocer el papel activo que ellas quieren y pueden cumplir para vivir una experiencia positiva y satisfactoria.


Introduction : Enhancing women's participation during pregnancy and childbirth is in line with the call of the World Health Organization and is linked to effects on user satisfaction, clinical health outcomes and better management of health care providers. Objective : To discover women's needs for participation in decision making during pregnancy and childbirth. Methods : Secondary analysis of a descriptive qualitative study with hospitalized women from the puerperium service of two hospitals in Santiago, Chile. The data analysis was carried out using the method proposed by Grounded Theory. Results : Twelve women participated in two focus groups. The relational analysis showed that participation in the reproductive process is interfered by two groups of factors linked to cultural meanings and to the violation of women's rights. Conclusions: Women's participation in clinical decisions during pregnancy and childbirth is still scarce and power is still held by health professionals, perpetuating practices of institutional violence. To advance in obstetric practice centered on women, it is important to recognize the active role that women want and can play in order to have a positive and satisfactory experience.
